The Greater Mercer Public Health Partnership (GMPHP) is calling on residents of Hopewell Valley and the greater Mercer County area to participate in the 2024 Community Health Survey. This anonymous survey is a crucial opportunity for community members to share their experiences, opinions, and priorities, helping to shape the future of local health programs and services.
The GMPHP, which completed and updated the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) in 2021, unveiled these plans in March 2022 with the support of the Community Advisory Board. Now, they are seeking fresh input to guide their ongoing efforts.

The survey is confidential and does not request names or identifying information. It is available in eight languages: Arabic, simplified Chinese, English, Haitian Creole, Hindi, Portuguese, Spanish, and Yiddish. Past survey results have led to impactful initiatives such as smoking prevention campaigns for youth, healthy cooking classes, expanded transportation options, and mental health training for first responders.
The GMPHP is a collaboration of three hospitals, eight health departments, Mercer County Human Services, and 70 not-for-profit organizations. Their mission is to measurably improve the health of residents in the Greater Mercer County community.
